Having key documents available helps families request VA burial and memorial benefits promptly. This checklist covers the documents most commonly needed — requirements may vary depending on the specific benefit requested.
DD-214 or Equivalent Discharge Document
The DD-214 (Certificate of Release or Discharge from Active Duty) is the most important document for requesting VA burial benefits. It confirms the veteran's service and discharge status. If the DD-214 is not available, it can be requested from the National Archives.
Death Certificate
A certified copy of the veteran's death certificate is required for most VA burial benefit requests. The funeral home typically obtains certified copies on behalf of the family.
Proof of Relationship
For benefits requested by a family member, proof of relationship to the veteran may be required. This may include a marriage certificate, birth certificate, or adoption records, depending on the relationship and the benefit requested.
VA Claim Number (If Applicable)
If the veteran was receiving VA compensation or pension at the time of death, the VA claim number may be needed for certain benefit requests. This number can be found on VA correspondence.
Requesting Missing Records
